By FictionFan Another beautifully illustrated book published by the British Library, this makes a fine companion piece to London: A Literary Anthology, which I reviewed a few months back. This time the focus is on food, with extracts from many familiar and not-so-familiar authors. There is a mix of both poetry and prose, grouped together under headings such as: The Art of Hospitality, Love Bites, Childish Things etc. In each section, the extracts go roughly from older to newer – for example, Dazzling All Beholders runs from Robert May writing in the 17th century to F Scott Fitzgerald in the 20th. As well as giant literary figures – Dickens, Flaubert, Proust and his famous madeleines, DH Lawrence, et al – there are food writers, such as Brillat-Savarin and Hannah Glasse.Most of the extracts are fairly short – no more than a couple of pages, and to be honest I didn't find them quite as mouth-watering on the whole as I was expecting. Often the pieces are more about things associated with food, rather than food itself – restaurants, dining rooms, dinner companions. The balance is very heavily weighted towards older writers, with very few, if any, contemporary writers making an appearance. I think the most recent extract is from about the 1930s. This may be for copyright reasons, at a guess, but it means that none of the exciting food writers of the last few decades are included, nor any modern literary writers.Without a doubt my favourite section was Childish Things, taking me back to many books I loved. Ratty's picnic from The Wind in the Willows, the Turkish Delight from Narnia, lashings of ginger beer courtesy of Enid Blyton, and Heidi's first taste of toasted cheese – all great scenes that really have lived in my memory since childhood. There's a section on fabulous feasting, with lists of enough dead animals to make a vegetarian faint, and I was glad to get from there to Simple Pleasures, on such delights as tea and hot, buttered toast. And Distant Times and Places brings us travellers' tales, from Gulliver to Captain Scott.The book is beautifully illustrated, if not quite so lavishly as the London anthology. Most of the illustrations come from the British Library's own collection, and they are often the specific original illustrations that match the text. Each extract is headed with a short introduction giving the date of writing, which I appreciated, having remarked on the lack of this information in the London book. The list of illustrations is at the back of the book, so requires flicking backwards and forwards if the reader wants to know more about them. The physical quality of the book is wonderful. The cover is gorgeous and pleasingly tactile, the pages are printed on high quality paper and the font and layout are clean and clear. A book that would make a great gift for any food-lover, the more adventurous of whom might want to try out some of the recipes in the final section. I'm not sure I want to eat Alexandre Dumas' Arab Omelette (made from ostrich and flamenco eggs) but Emily Dickinson's Gingerbread would go nicely with George Orwell's Nice Cup of Tea...NB This book was provided for review by the publisher, The British Library.

About the Author Michael DeVault was born in Mississippi and grew up in Louisiana and Arkansas, which gave him a strong grounding in the rich musical and literary traditions of the South. He worked as a journalist for more than twelve years, covering politics and the arts for local and regional publications while he also worked on his novels. A two-time finalist for the Pirate's Alley Faulkner Society Gold Medal (Novel-in-Progress, 2002, Anything But Ordinary; Novella, 2008, The Patriot Joe Morton), Michael's fiction draws on his youth to weave tapestries of intensely believable characters, finely honed plots, and imagery and symbolism inspired by the great southern writers, all wrapped into a package by clean, sharp prose. Michael received an MFA in Creative Writing from Lindenwood University in 2013, and when he's not writing, he teaches college writing and English.

Kwashnak Bookshelves groan under the weight of numerous coming of age stories as well as numerous odes to various cities around the world. Few of those loved cities have suffered such a major, and publicly recorded disaster as New Orleans has. The city has been written about so much that many of us feel we know her streets and denizens well despite never setting foot on her streets. In Anything But Ordinary, Michael DeVault weaves just a slice of the Crescent City with the story of a twelve year old boy sentenced to community service in a nursing home for a summer. There he encounters a half-dozen eccentric elderly residents who become and extended family to him, and those who work at and administer the home. The residents of "the Row" come from central casting of off-kilter and free-spirited characters who find their bond in their differences. More importantly they are the living reflection of the city in which they live - reveling in the fact that they are different and unique and flaunting the conventions of `the rules'. We are not treated to an extensive travelogue; most of the story takes place inside the nursing home, or the apartment the twelve year old protagonist Billy shares with his mother. Rather we see the city through the character traits of the row's residents. Billy's an outsider from Minneapolis who learns to love his new friends, and by extension his new home. Into this world comes another out-of-towner, the home's new administrator and his young daughter. The administrator is the antithesis of New Orleans - brash, demanding, forward and forceful, while his daughter is caught up in her own world in a desperate search for her long gone mother and is a character to fit right into the city. The clash of cultures, just like the clash of nature and the government's delayed reaction with the city leads to disaster and heartbreak. However, in the routine of the day-to-day and the ordinary, we come to love a city that is anything but.

If your kids like "Where the Sidewalk Ends", they'll probably like these poems too.C.J. Dennis is Austrailian and this was written in 1917, every once in awhile, there's an odd word, but the poems are still very fun and humorous today, especially for the child who uses their imagination. THe illustrations are cool too, they come across great on the Kindle and if you want to see them in color, Gutenberg has them in color in the html format.

Cute mystery By C Thilmany The second book in the Daphne Martin Cake Decorating Mysteries puts Daphne into the uncomfortable situation of investigating the death of an acquaintance. She'd been at the hospital visiting when Fred passed away and his mother begs Daphne for help since she has experience at this type of thing. Of course her own family would prefer that she stayed out of it. More than this initial mystery is solved in this story.A large part of the story is built around Daphne's business of making and decorating both cake and candy. At first the step by step process for making a gum-paste character seemed a little much, but I began to appreciate some of the ideas and instructions provided in the story as other things were made.The mystery itself was well done. Information is gathered in a fairly relaxed manner as Daphne goes about doing other things while taking time to ask questions. We're given a number of possibilities but the protagonist isn't easy to guess before the author decides it's time.I found it a light, enjoyable read.

My nerves needed the distraction." -- From Dead PanThe breezy second installment of the Daphne Martin cake decorating mystery series, Dead Pan cooks up a delightful culinary cozy mystery that is arguably even better than the first scrumptious book, Murder Takes the Cake.Dead Pan features some of the quirky, memorable charcters from the first book (e.g. Myra, China York, Belinda Fremont and her award-winning guinea pigs, etc.) and introduces some new ones.In this installment, brain-damaged Fred Duncan gets violently ill at the Brea Ridge Pharmaceuticals Christmas party along with dozens of others; a doctor in attendance just happened to have a vaccine on hand that counteracts the Campylobacter bacteria--curing everyone except for poor Fred, who ends up dead.Was the breakout mere coincidence? Or did someone have murder on their mind?Nosy but likable cake-decorator Daphne Martin finds herself once again in the middle of a police investigation, much to the dismay of her sister, Violet.I ate up Dead Pan and loved every bite. I can't remember being this entertained and delighted with a cozy series--the first installments of the Coffeehouse Mysteries, maybe?It was interesting how the author incorporated dialogue between Daphne and real-life Food Network judge and sugar artist, Kerry Vincent. I knew who Vincent was, so her "cameo" was a pleasant surprise (although I'm not sure how Ms. Vincent would feel about being fictionally inserted into a mystery!).I can tell you this: I'm very much looking forward to reading book 3 in the Daphne Martin Cake Decorating series, Killer Sweet Tooth, which is supposed to come out sometime in 2010. Author Gayle Trent has a knack for creating fun characters, believable dialogue and gentle mysteries, and I'm thrilled I discovered her and the Daphne Martin culinary cozy mystery series!-- Janet Boyer, author of Back in Time Tarot

11 of 13 people found the following review helpful. Dead Pan? More Like Dead Wrong! By Deshawn Taylor Ultimately, I was displeased with this book. If you take out the detail given to baking the cakes, the book would be incredibly short. Great detail was given to develop the story while the solving of the crime was summed up in 3 or 4 pages. Many loose ends were left untied upon the conclusion of the book. What ultimately killed Fred? Why did Daphne's uncle warn her to stay away from Ben? Also, several characters had no real purpose for being in the story. What was the point of reintroducing Belinda Freemont and her guinea pigs? Why was the Sav-A-Buc store manager's brother even mentioned? I think that this book should have been free instead of Murder Takes The Cake. It wasn't worth the $9.99, especially since the last 25% of the book was recipes and excerpts from other books. If I wanted dessert recipes, I would have purchased a cookbook.

About the Author Arguably the greatest English-language playwright, William Shakespeare was a seventeenth-century writer and dramatist, and is known as the Bard of Avon. 9 of 11 people found the following review helpful. Contemporary History By Randy Keehn I have tried to abide by a discipline to read at least one Shakespearean play a year. I rarely if ever have reviewed any of them because they are so picked over that I don't feel that I have anything to add. However, it was with a good deal of anticipation that I stepped away from the renowned to the obscure by reading "King Henry VIII". I say that because it was the subject that interested me and I admit that it made quite a difference reading a play about someone I knew something about. This, in turn, gave me pause to consider many things about "King Henry VIII" that I would never have even known to consider about his other histories. It's not that I read many of his other histories but those that I had read left me completely at the mercy of the Bard for any sort of perspective or overview.What I found most interesting about "King Henry VIII" is the limited scope that the play covered. Let's face it: even today Ol' King Henry VIII is a treasure chest of plots and subplots. Yet Shakespeare treated his subject with a great deal of respect and, essentially, rewrote history before it was even written. The four or five main characters (with the exception of Cardinal Wolsley) all come across in good light. Perhaps Ann "Bullen" is a bit empty-headed but certainly Henry, Anne of Aragon and the lesser known (outside of the Church) Thomas Cranmer are all noble through and through. There is little of the bawdy, glutonous Henry that history has given us. You would guess that Shakespeare would have done great things with such a subject but he didn't and the question that I asked myself was; WHY?I have not studied anything about this play. I prefer to always see what the play says directly to me before being told by others as to what it means. Thus I may be stating the obvious but I came away with the conclusion that "King Henry VIII" was written for Shakespeare's Quenn Elizabeth. It was pretty obvious when reading Cranmenr's final speech and it put everything into perspective. All that went before were noble and all that came after were not even mentioned. The "noble" divorce of Henry and Ann of Aragon was necessary so that Henry have the proper opportunity to sire Elizabeth. I'm not sure enough of my timetables to know if Elizabeth I was still alive when this play came out but even if she weren't it would still be the Bard's tribute to her.I had come to expect that most of the "good stuff" of Shakespeare (with the exception of "King Henry V") was to be found in his tragedies primarily and comedies next with the histories coming up a poor third. However, I kept my pencil busy underling passages in "King Henry VIII". It is a good play, it's not "MacBeth", but it's a good play. I'd have considered rating it 5 stars but then we'd have to rate some of the rest as 7's 8's and 9's. Most people who'd take the time to read Shakespeare probably already have a pretty good working knowledge of at least the popular history of the King with six wives. Read it for yourself and I think that you, too, will find that history is better understood when you are already familiar with the subject.

One of the better Shakespeare's you haven't read By John Cullom One of the more interesting Shakespeare-s in that it closes out his career with a history. With the Arden, you get a lot about the performance history, which of this play is especially important. H8 was a big spectacle, and that was important to the success of the play. We have movies and the like, so now this isn't performed as much, but it was one of the most popular plays over the history of Shakespeare for the spectacle itself.There's also huge interest if you're reading Wolf Hall or watching the Tudors. Everyone has a different take on these characters, and the same ones are in all 3. Wouldn't you like to know what Shakespeare's take was? Sure you would . . . sure you would.

Multiple editions By Carvet ... the reviews for King Henry VIII by William Shakespeare (and all their other books as far as I can tell) as if different editions have the same content - obviously in the case of classics that is far from true.... 3 editions of Henry VIII at this time: (1) Hardback edited by Gordon McMullar published in November 2000 (2) Paperback edited by Jay L. Halio published in September 2000 (3) Paperback edited by R. A. Foakes published in February 1998Their editorial reviews describe ALL 3 of these editions as "This is the first fully annotated modern-spelling edition of King Henry VIII to appear for over a decade and includes up-to-date scholarship on all aspects of the play, including dating authorship, printing, sources and stage history." I don't think so! The reader reviews don't distinguish the editions but they are the same reviews posted for the different books. I wish I could contribute the answer but I am still trying to figure it out -- in the meantime, purchase cautiously or you may be disappointed.

By Jon I only bought this book for the meditation advice but was suprised at the great introduction to Zen Buddhism, very interesting indeed.This book has some very information about how to train and free your mind and I'm looking forward to trying some of the exercises here. One part of the book which keeps making me going back for more is the chapter on 'Zazen Poses'. There's some great zen meditation poses included here and all of these positions are explained extremly well, so much so that I actually understand them!Well laid out book, looking forward to progressing through the rest of the various positions and exercises here.

An excellent read on Zen principles By Tetay This is a well written guide for anyone beginning their journey or simply seeking to learn more about Zen Buddhism. The author takes you step by step through understanding the history of it, implementing the concepts in your daily life, meditation and also provides resources for finding the right teacher to continue your journey. I really enjoyed that the basic concepts are presented to the reader however the author does not try to confine Zen Buddhism to a specific regime or ideal. The concept of Zen is open and interpretative which I like. This is was a great introductory read.

I also liked the mindfulness section, cutting through the hype and laying out exactly what it's all about and how to start practicing. Recommended.

By romancereader I typically stay away from books not set in the U.S. because I don't always relate well to the language differences. But because this book being written by an American, just being set in Rome, I thought I'd give it a shot. The Playboy of Rome was a sweet story of a girl who is sacrificing herself to support her foster sister. I liked the premise of the story - girl travels to Rome, boy doesn't expect her, makes assumptions. Dante and Lizzie had chemistry from the beginning and it permeated their relationship throughout the story. I did find it frustrating that Lizzie continued to believe Dante was a playboy when he had done nothing to earn that reputation while around Lizzie. Dante's reasons for avoiding commitment were equally compelling and I really felt for Dante as he struggled to fight his feelings for Lizzie because of his belief that he was destined to live alone. In true Harlequin form, the story was tightly written and well edited. As expected, Dante and Lizzie faced a situation that threatened their happiness. I felt the reason for their estrangement was a bit contrived and one or two simple conversations would have solved their miscommunication. All in all, The Playboy of Rome was a sweet read with an expected happily ever after. Recommend it to anyone who is looking for a quick read with a hero to swoon over!

It is a rather short book that I was able to read in just a couple of hours while my family was sleeping on a weekend morning. I read a lot of different kinds of books and every once in a while I like to read a nice, easy romance novel. This book wasn't perfect and some of the events in the story felt a little too convenient but it was an enjoyable story nonetheless.Lizzie has had to deal with a lot in her lifetime. She leaves her job and life behind in America to take a chance in Italy working with a chef on a television show. She is determined to make things work because the person that means the most to her in the world is depending on her. When she arrives at the restaurant, she does not receive the warm welcome that she had expected. The chef that she had plans to work with is no longer able to work and has been replaced by his grandson, Dante.Dante is not impressed with the idea of training Lizzie or the television show. To make matters worse, his grandfather promised Lizzie a place to stay while she is in Italy and he finds himself sharing his apartment with her when they are not working.I liked the chemistry between Lizzie and Dante. They have both lost a lot and really seem to understand each other. I found the individual characters of Lizzie, Dante, and Dante's family to be very likeable. Everyone in this story has lost a lot in life and has learned to deal with things the best that they know how.I would recommend this book to anyone looking for a quick contemporary romance. This was the first book that I have read by Jennifer Faye and I do plan to keep my eyes open for future titles by this author.I received a copy of this book from the author for the purpose of providing an honest review.

Sweet Romance set against the Italian countryside By The Sassy Bookster 3.5 StarsIn The Playboy of Rome, the Italian countryside provides a backdrop for romance when an aspiring chef faces off against a reluctant restaurateur over a reality show! Lizzie Alder has given up everything for the opportunity to film her reality show at Ristorante Massimo and with no back up plan, she is determined to convince Dante DeFiore to honor the agreement she entered into with his grandfather.Dante is in the process of finding a buyer for the restaurant and the signed agreement provided by Lizzie means putting the plans to sell on hold and stepping in for his grandfather on the show. Dante and Lizzie find themselves getting to know each other and dealing with the heat between them (not generated in the kitchen!) Both Dante and Lizzie have different backgrounds but share a lot of similarities. Lizzie was a foster child who was abandoned by her own mother and shuffled from home to home until she aged out of the system. Her experiences have taught her not to trust in anyone except herself and her foster sister Jules. Dante on the hand grew up within a large Italian family but was mostly ignored. Both crave love and acceptance and Lizzie finds the family she craves in Dante’s and also helps to heal the rift between Dante and his family. A few times, it felt like Lizzie was too hung up on her past as a foster child and just could not get past it, but she found the courage to overcome her issues and break through Dante’s resistance. Even though the characters were in the culinary industry, I liked that the story was not overwhelmed by cooking, but rather the scenes where they cooked were just enough to enhance the story.The Playboy of Rome was an enjoyable read with a strong and brave heroine, one who overcame adversities to achieve her dreams and find love.*I received this book for free from the Publisher in exchange for an honest review. Wasted Money, Temporary Placebo Gains, Deliberate Deception By Loyd Eskildson The book reports how/why doctors continue to used medical practices, sometimes for decades, that are later shown to be of no benefit to their patients. Time and again, we 'learn' that a new practice will help extend life, then that it does not. (Estrogen-replacement therapy after menopause, placement of coronary stents to open narrowed/blocked coronary arteries, cupping - topical suction, lobotomy. Vioxx, flecainide, losartan, routine mammography for women in their 40s) The authors assert that while there are instances of good hypotheses failing to live up to expectations - such should happen in a lab/controlled clinical trial - not in clinical medicine. They believe medicine can do a better job recommending practices that actually work.Medical reversal/disbelief is strongest when involving how people feel - eg. vertebroplasty, coronary stents (for the first three years), arthroscopic knee surgery to repair degenerative meniscal tears (700K/year),Seven states in the career of a medical innovation,' pre-Evidence Based Medicine (EBM), per John McKinlay: A 'promising report' in which a medical innovation is publicized based on its promise. 2)The innovation is adopted. 3)Patients and payers accept the innovation as standard. 4)Data begin to become available - however, those supporting the innovation come only from insubstantial studies that support the innovation in the most superficial way. 5)The RCT makes an appearance. 6)Then denial, if its use is not supported - entrenched interests deny the innovation may not be effective. 7)Acceptance.While EBM has filled journals, they often avoid big questions or favor one side such that they are not useful.Surrogate end points are objective ones that can be easily and directly measured (eg. improved bone density vs. fracture rates, lowered blood pressure vs. decreased rate of strokes, antiarrhythmic drugs decreasing the number of premature ventricular contractions but not survival after a heart attack, hospitalization rates). However, they are invisible to the patient.Summarizing to this point, we have seen reversal for practices meant to make us live longer when evidence supporting that practice was weak or flawed - this included times when the evidence relied on surrogate end points. For practices meant to make us feel better, we have also learned how powerful the placebo effect is, and noted reversals when treatments were later tested using appropriate controls, such as sham procedures.Screening recommendations have also be reversed. In many reversals involving screening are the worst kind - they are performed on healthy people and thus affect an enormous number of them who simply want to stay that way. An ineffective screening test can turn millions of healthy people into patients. Recommendations for PSA tests, mammograms for women in their forties, Pap smears, etc. have all been revised.In studies of CT-scan screening for lung cancer in those between ages 55 and 80 with a heavy-smoking history is the only screening test to-date that makes you live longer - and even here am many as 96% of abnormal findings are false alarms. In studies of colorectal cancer with 30 years of follow-up, for every 10,000 people, 192 die of colon cancer in the unscreened arm, vs. 128 in the screened arm. However, looking at overall mortality, 7,109 out of 10,000 die in the unscreened group vs. 7,111 in the screened group - not a significant difference.Most experts agree that all cancer screening leads to some amount of over-diagnosis. When we screen for prostate cancer, we end up treating about 40 cancers for every cancer that will kill. For mammography, the best studies suggest that if a mammogram finds breast cancer and it is treated, there is a 13% chance this will have saved a life.Each year in America, we spend hundreds of billions on screening tests and their downstream costs. In 2011, two articles were published, finding that gown-and-glove precautions did not decrease transmission of VRE or MRSA in ICUs - an intervention generating additional costs w/o benefits. However, gown-and-glove precautions were adopted largely on the basis of 'single-center, before-and-after studies. The proof from such a study tends to be tenuous because that center may be idiosyncratic - a unique demographic of patients or bacteria, or an unusually enthusiastic proponent who changes the culture in ways not reproduced elsewhere. Before-and-after studies are also problematic, and the initial data behind most systems interventions. However, one's intervention is never the only thing changed (Hawthorne effects) - and per the author, we have yet to see a very large, well-done randomized trial confirming that eg. a checklist is truly what makes the differences. Another - rapid-response-teams for cardiac arrests were demonstrated in the 2005 MERIT trial in 23 Australian hospitals to not improve patient outcomes. Yet, gown-and-glove precautions and RRT are widely accepted, despite not working.'Door-to-balloon' time was considered an important objective. A 2013 study found that between 2005 and 2009, door-to-ballon times were significantly reduced across the nation - the number of patients waiting longer than 90 minutes decreased from 40.3% to 16.9^. Mortality, however, was unchanged. Explanations - perhaps we were not targeting the right metric (instead focus on total time), or decreasing door-to-balloon time might be an example of diminishing returns. A final example - in 2001 a single-center unblinded RCT found that lowering blood-sugar levels to normal levels could improve survival in a SICU. (A novel strategy because one of the body's responses to critical illness is to raise blood-sugar levels.) This quickly became a new standard until a 2009 multicenter (42 hospitals) RCT found the practice increased deaths by 2.6 percentage points at 90 days.Prasad then points out that while doctors often recommend treatments that do not work, patients also do - on their own. Glucosamine and chondroitin ($700 million in 2004) - a 2006 RCT found no difference in pain; this was followed by a 2010 review of data from 10 different trials that reached the same conclusion. Echinacea - nearly 20% of Americans reported using Echinacea in the past 30 days, mostly to reduce the duration of cold symptoms. A 2005 RCT study concluded it did not reduce symptom duration; this was later supported by 7 randomized trials - only one of which reduced symptom duration compared to a placebo. (The authors also examined 12 studies that looked at Echinacea to prevent colds - none of the studies showed an effect.)Acupuncture is a popular treatment for pain, dating back over 3,000 years. An effort to summarize all research analyzing acupuncture published (Western Chinese, and Korean scientific literature) between 2000 - 2009 concluded there is 'little truly convincing evidence that acupuncture is effective in reducing pain,' and also enumerated a few examples of acupuncture causing real harm.Use of multivitamins in a 2009 study of 161K women found no link to declines in cancer, heart disease, or mortality; a 2013 review of randomized trails found no clear benefit on overall survival, heart disease, or cancer.Recent analyses of randomized trials found that calcium and vitamin D supplementation does not reduce risk of fractures among healthy women, and the U.S. Preventive Services Task Force recommends against the supplements. On the other hand, we've known that calcium and vitamin D supplementation increases kidney stones, and a 2010 group of researchers added heart attacks as another potential side effect.When it comes to dietary habits, studies are less like science and more like an opinion poll. Most diets are never tested in a randomized trial, and when they are, the main outcome of study is usually short-term weight loss. In PREDIMED (an RCT evaluation of the Mediterranean diet, using randomly assigned patients at high risk for a cardiovascular event) found one had to treat about 90 people with the diet for 5 years to prevent one stroke - yet no mortality differences.In 2005 John Ioannidis wanted to measure the proportion of important findings in medicine that were later contradicted. He started with studies referenced over 1,000 times published during the years 1990 to 2003. Of those, 45 found a medical intervention effective, and of those 16% were later found ineffective, another 16% less effective than initially believed, 44% supported in future studies, 24% never tested again. The authors did their own research, using articles in the 2009 NEJM - of the 35 studies examining current standards of care, 46% showed current standards as ineffective. They then extended their study to NEJM articles between 2001 and 2010. Reversals were found in 40%, 38% reaffirmed the benefit of a new practice, and 22% were inconclusive. A project of the British Medical Journal Clinical Evidence completed a review of 3,000 medical practices and found 35% effective, 15% harmful/unlikely to be beneficial/tradeoff, and 50% of unknown effectiveness.Tracking citations to three major practices found not to work: beta-carotene to prevent cancer, estrogen to prevent Alzheimer's, and vitamin E to lower cardiovascular risk, researchers found that 10 years passed before the research community stopped referencing the flawed practice.Original analyses of prescribing estrogens for post-menopausal women was cofounded by the self-selected nature of those originally taking the supplements - less likely to have a family history of heart disease, be hypertensive, have diabetes, or smoke; they also were younger, drank more alcohol, and consumed more saturated fats.A 2012 paper titled 'Empirical Evaluation of Very Large Treatment Effects of Medical Interventions' (Pereira, Horwitz, Ioannidis) examined the proportion of medical trials showing a very large treatment effect (eg. 5X or more). They looked at over 228,000 trials and found only 9% did so. Topics with large effects were less likely to be about mortality and more likely to be about a laboratory value. They then looked at other studies addressing the same questions as those demonstrating the very large treatment effect - 90% of the time, the large treatment effects got smaller when one looked at other studies, thus strongly suggesting the largest magnitude of effect ae more likely statistical flukes. Across all the studies, only one had a large effect on mortality - a method for oxygenating blood of newborns who cannot adequately breathe on their own.RCTs have intrinsic error rates, added to by early termination of RCTs, publication bias,Industry-sponsored studies are 4X as likely to reach a positive conclusion; they are also less likely to be published/presented, or published after a delay. When companies hold back evidence (device-makers, drug formulators), the medical literature becomes selectively drawn from a much larger pool. Tamiflu was thought to prevent transmission of the flue, decrease hospitalizations, and save lives - an RCT found it decreased flu symptoms by less than a day - while causing nausea and vomiting, did nothing to prevent transmission of the virus, reduce hospitalization or deaths. Previously, Roche (Tamiflu manufacturer) had published its own meta-analysis, and found large benefits. Their study included only those proven to have influenza (rather than those with influenza-like illness) and focused on those receiving the drug early - different than typical use.The FDA cannot consider cost as part of its deliberations, nor relative efficacy. Then, in the early 1990s, the FDA pioneered an accelerated-approval program - allowing drugs for serious diseases with few treatment options to gain approval by showing benefit on a surrogate end point reasonably likely to predict clinical benefit. The drug is then given a period of time to prove it benefits a more important end point. However, after a drug is approved, it is much harder to get participants to enroll in a study (no one will pay for a trial if there is no prospect for large returns) and after 20 years one-third of these post-approval confirmatory studies had not been completed.What is also not acceptable is for a drug company to develop a drug for one indication and then market it for something else - gabapentin is an egregious, but not the most recent, example. In 1993, under the trade name Neurontin, it was approved to be used for the treatment of seizures when combined with other drugs. Its maker, Parke-Davis, then marketed it for eg. bipolar disorder, pain disorders, etc. AND as a single agent for seizures - despite having previously being rejected for the latter by the FDA. A $430 million settlement followed, but over 20 years later gabapentin is still not approved for most of the off-label uses for which it was initially promoted. (Another reversal-prone situation.)A bias to 'act now, data later' also contributes to reversal, rapid reporting on new technology is another, along with direct-to-consumer advertising (we're one of only two nations {New Zealand} that permit such). An evaluation of DTC advertising found 10% of those with 'adjustment disorder' symptoms and did not request a medication received one, vs. 39% of those making a general request and 55% making a brand-specific request. Between 1999 and 2005, DTC advertising increased from just under $1 billion to over $4 billion. Another - a medical school emphasis on 'reductionism' (understanding the mechanism by which a drug purportedly works), instead of empiricism (does the drug work).

Needs to be required reading in medical school By Miss Barbara Ending Medical Reversal is a book that should be required reading in medical schools. It is eye-opening for both the medical and lay person in that is asks the big, hard, dirty questions. I’ve been around long enough to have seen so many of the “Cures du’jures” that are touted to save mankind with its neoteric food regimens or its latest pill only to have these procedures negated in the next year or two. “Sorry, my bad” should not be the modus operandi in the medical field.Basically, medical reversals are described as medications, procedures or tools that become widespread and then quickly become out of vogue when it’s determined that they have no strong or tough evidence that they actually work, in fact many have been proven to cause harm. This book points out that many of the most popular drugs from the formulary in the last 10 years were indeed successful in moving a patient’s “numbers” into a more acceptable range BUT they did next to nothing in improving death rates for the disease or syndrome they was prescribed for.There are many books like this out today that rail at the medical profession, big pharma, and the heath care insurers for their quest to improve diagnostic numbers and not improve the patient’s state of health – what makes this book different and commands people to sit up and listen is that the publisher is Johns Hopkins University Press. Yes, the big boys have entered the game.This is a very readable book for the professional and layman alike. There are a lot of first-hand accounts included that give the book a personal characteristic and reminds the reader that we’re talking about real people’s lives and how health care choices affects us all. I can’t tell you how happy I am to see this book published with the pedigree of knowledgeable authors as Dr.Vinayak Prasad, a cancer specialist, and Dr. Adam S. Cifu from the University of Chicago.

14 of 16 people found the following review helpful. A plea for evidence-based medicine By Phelps Gates This is one of the best books on medicine that I've read in a long time. The format suggests that it might be aimed at professionals, and it does have suggestions on how to improve research and medical education, but the intended audience is the general reader, and there's nothing in the book that's not easy to understand. And the authors are careful to summarize and reiterate their important points.The book discusses a large number of case studies where it turned out that what doctors were doing was wrong. Some of them have been written about a lot (estrogen replacement, PSA screening, the Neurontin scandal, etc., etc.), and many others less so. What makes the book worth reading is that it goes into great detail, explaining exactly what went wrong, and how, and how things could have been done better. Sometimes the problem was doing things that ought to have worked in theory but didn't in practice. Sometimes it was relying on studies that were poorly designed, with subtle biases (of many, many types). Sometimes it was deliberate malfeasance, even criminality, on the part of drug company executives. One of the things that medical students are taught (or so I've been told) is that you should never let a patient hear you say "oops!" and there have, alas, been quite a number of "oops" moments in medicine.The authors make a plea for "evidence-based" medicine. There's something of a tug-of-war in medicine, as in some other scientific disciplines, between reductionism (figuring out exactly what's happening) versus empiricism (discovering what actually works in practice). With reservations, the authors come down firmly on the side of empiricism: it's the job of the clinician to find out, and use, strategies of treatment that have actually been shown to work.Medical news stories ("a new study shows") appear almost daily in the press, sometimes in garbled form. Just this morning (October 8) there's a study that the headlines say calls for "doubling or quadrupling" daily exercise to prevent heart failure (from 30 to 120 minutes a day). Is this a well-designed study? One might surmise that people who are able and willing to exercise two hours a day are already in better condition. Has this been controlled for? Or do we have another medical reversal waiting to happen? It's hard to say, but reading the book has certainly increased my own ability to evaluate the latest breakthroughs and make more intelligent choices.
